What Idaho requires before you can deliver home and community-based waiver services, in the order the state actually requires it. Every claim links to the official page it came from.
Provider rules change often — confirm fees and deadlines against the linked sources before you rely on them.
Idaho's process is unusually concrete: your certification stands or falls on a policy and procedure manual that contains everything IDAPA 16.03.21 requires. That is the deliverable — not a narrative application, not an interview. Write the manual against the rule, section by section, and use the department's own initial-and-renewal checklist as the table of contents. Two other things shape the runway. Certification comes before Medicaid, so budget for a second, separate enrollment through Gainwell once you hold the certificate. And your geography is baked into the certificate: adding a location in a different region, moving between regions, or changing ownership all require a new application rather than an update.

SourceYou register the agency with the DHW Background Check Unit to receive an ID, and every employee accessing or working with participants must then hold a criminal history clearance under that ID.
SourceDHW publishes the approved Assistance with Medication trainings for DDA and ResHab providers. Idaho's technical colleges and universities are approved in rule; other providers — including Divinity Health Institute and Relias course series — carry individual approval dates, and at least one is approved subject to an annual update.
SourceIdaho deems the effective date of enrollment to be the date the completed and acceptable application is received by DHW or Gainwell. Exceptions must be requested in writing with justification, and the requested date must be covered by the license or certification you submitted.
SourceAll claims must be submitted to Idaho Medicaid within twelve months (365 days) of the date of service, with a narrow exception for Medicare crossover claims. That window is worth knowing before your billing process is live rather than after.
SourceOnce certified, you must submit a request for renewal of certification 60 days before your certificate expires. Any addendum, update, or change to the approved application requires an addendum submission.
SourceNew ownership, adding a location in a different region, or moving locations from one region to another all require a new application — not an amendment. Submit the updated scope-and-service policy for a region change, and the updated organizational chart for new ownership.
