How to Become a Medicaid Waiver Provider in New Jersey

What New Jersey requires before you can deliver home and community-based waiver services, in the order the state actually requires it. Every claim links to the official page it came from.

Becoming a waiver provider in New Jersey

Two decisions shape the New Jersey application, and both come before you fill anything in. The first is which services you will offer: the service you select generates the required-documents list the rest of the application is built on, and DDD tells you to save or print that list because it is what you will be held to. Services can be added later, so it is reasonable to start narrow. The second is how you will be paid — as a Medicaid provider, whose Combined Application is submitted to Molina, the state's fiscal agent, or as a business entity or individual practitioner applying through the Department of Human Services' Fiscal Intermediary. Those are different routes through the same portal. Two services carry an extra gate worth knowing early: Day Habilitation requires provisional Day Habilitation certification before approval, and Support Coordination is subject to a conflict-free policy that may rule out combining it with your other services.

Selecting services in the Combined Application creates the required-documents list — which must be saved or printed — and produces a verification code. You can choose multiple services, and add more later.

How to enroll in New Jersey

Get an NPI, review the service definitions and qualifications, choose your services in the Combined Application on the Fee-for-Service Provider Portal, then submit — to Molina for the Medicaid route or through the Fiscal Intermediary.

    Medicaid providers submit the Combined Application to Molina — click "Apply Through Medicaid." Business entities and individual practitioners apply through the Fiscal Intermediary instead.

    The application packet is downloaded, completed, and submitted by mail to the Molina Medicaid Solutions Provider Enrollment Unit, P.O. Box 4804, Trenton NJ 08650-4804.

Registration with DDD plus, often, Medicaid approval

Providers must be registered with DDD and, depending on the service, must also be an approved Medicaid provider. Payment comes either through Medicaid's fiscal agent or through the Department of Human Services' Fiscal Intermediary.
